RM 01340.041 Examining the Date
The date the request was signed must be provided on all third-party requests. The date is usually provided next to the signature either in the blocked area at the end of the Form SSA-7004-SM-OP1 or at the end of the correspondence.
NOTE: The date the request was signed is not required on first party requests.
A third-party request received from another individual, agency or organization is not acceptable for processing if not dated by the requester.
C. When to use
Use this procedure when examining the date the requester signed the Form SSA-7004-SM-OP1 or correspondence.
Examine the date the requester signed the Form SSA-7004-SM-OP1 or correspondence. Take the following actions:
Determine if the requester is a third-party request.
If the third-party request is received in SSA no later than 60 days from the date the request was signed, sort for data entry.
If the third-party request is not received in SSA within 60 days from the date of signature, return the original request under cover of Form SSA-L220. Check box 6.
