RM 03252.001 Block ERISA Requests Received from Receipt and Dispatch (4402)

  1. A. 

    Receive ERISA requests for deferred vested rights pension information.

  2. B. 

    Accumulate ERISA requests on a weekly basis.

  3. C. 

    Keep a running count of all ERISA requests received and forward to the Records Utilization and Services Branch (RUSB) at the end of each month.

  4. D. 

    Block ERISA requests as follows:

    1. 1. 

      Not less than 10 nor more than 50 requests to a block.

      EXCEPTION:

      •  

        When one week's accumulation of requests is less than 10, continue accumulating requests a second week. If, at that time, there are still less than 10 requests, assign a block number to the requests on hand and further handle as in “D.2.”

    2. 2. 

      Assign ERISA requests block numbers in accordance with the following:

      1. a. 

        Prepare pre-numbered control sheets listing 2 digits block numbers beginning with 01 through 99. Repeat block numbers after assigning block number 99.

      2. b. 

        Check control sheet for the next block number to be assigned.

      3. c. 

        Enter block number in proper space on Form SSA-4352, ERISA Pension Notice Request Transmittal.

    3. 3. 

      Enter the number of requests being assigned in proper space on Form SSA-4352.

  5. E. 

    Issue ERISA requests for Examining.
