RM 03253.002 SSA-L99-C1, Notice of Potential Private Pension Benefits
The Social Security Administration is required to furnish individuals (and dependents
or survivors) deferred vested benefit information upon request and automatically upon
application of a claim for social security benfits
SSA provides individuals the exact information that was submitted by the plan administrator.
This pension information is furnished on Form SSA-L99-C1, Notice of Potential Private
Pension Benefits. See Exhibit 2.
The following information from SSA's files is furnished on the SSA-L99-C1:
Plan number (Twelve digit number)
NOTE: For identification and retrieval purposes SSA combines the three digit plan number
and nine digit EIN, respectively, furnished on the Schedule SSA to form this twelve
digit plan number.
Plan Administrator's name and address
NOTE: IRS maintains current name and address files of all plan administrators for specific
pension plans and furnishes weekly updates for our magnetic tape file of plan administrators.
Plan Administrator's identification number (Nine digit EIN).
Deferred vested benefit information:
Year Reported (Plan year, calendar or fiscal, for which the information was submitted.
Amount (This is periodic payment to be paid monthly, annually, etc.).
Type of Annuity code (See back of SSA-L99-C1).
Payment Frequency code (See back of SSA-L99-C1).
Units or Share (Total number of units or share held at the time of separation).
Value of Account (Total value of units or shares held at the time of separation).
