      NOTE: This is a five position alpha/numeric field. The first two positions will be a constant numeric “00,” the second position will be an alpha character, and the last two positions will be numeric.

      Block Number 1 — SSN

      Block Number 10 — Surname and initials of SSN holder.

   4. 4. 

      Block Number 18 — Title and name of requester.

   5. 5. 

      Block Number 44 — Street address.

   6. 6. 

      Block Number 70 — City.

   7. 7. 

      Block Number 85 — State abbreviation.

   8. 8. 

      Block Number 87 — ZIP CODE.

2. B. 

   Do not key Forms SSA-4617-U2 with the following conditions:

      SSN not shown.

   2. 2. 

      SSN incomplete.

   3. 3. 

      Entire name or surname missing.

   4. 4. 

      Requester's name and complete address missing from designated blocks.

   5. 5. 

      Illegible information.
