RM 04902.001 Receive Forms 4029 (5874)

  1. A. 

    Receive Forms 4029, with related attachments, from the Internal Revenue Service. (Remove any finalized forms and handle in accordance with RM 04906.001.

  2. B. 

    Date stamp each Form 4029. Position the stamp to print over the pre-printed paragraphs in the body of the form. Do not stamp over data entered by the individual.

  3. C. 

    If a Form 4029 contains no possible SSN and insufficient identifying information to permit screening for an SSN, set the form aside for immediate return to the IRS office that submitted the form. Prepare and attach a “Routing and Transmittal Slip Form 41”, to each case. Indicate the IRS Office (Attention: Correspondence Section), and explain that the form 4029 does not contain a possible SSN and insufficient identifying information to assign a number. Release the case for mailing.

  4. D. 

    While date stamping, segregate the forms, other than those in C. above, with related attachments, if any, into the following groups:

    1. 1. 

      Group A — Form 4029 containing a possible SSN and not accompanied by a completed form SS-5.

    2. 2. 

      Group B — Form 4029, without a possible SSN, accompanied by a form SS-5 (signed or unsigned by the individual), or sufficient identifying information is supplied by IRS (at least the name, date of birth, and mother's or father's name).
