RM 05004.002 Handling Correspondence With Incorrect Social Security Numbers (SSNs)

A. INTRODUCTION

You may receive some correspondence involving incorrect SSNs.

B. PROCEDURE

Follow the instructions below.

1. Correct SSN Located

Use a dictated or form letter (if appropriate) as follows:

  • Tell the individual that the SSN furnished by him/her is not  an SSN or is not assigned to him/her or to the individual for whom he/ she is writing.

NOTE: Do not show the incorrect SSN. Give the correct SSN only if the Privacy Act (PA) permits.

  • Enclose a courtesy copy of our reply and ask the individual to return the copy, together with: The Social Security card that he/she has; or the card for whom he/she is writing about.

  • If the reply is made by form letter or stuffer, stamp or annotate any enclosures on which the correct SSN appears with the notation “PLEASE NOTE CORRECT SOCIAL SECURITY NUMBER.”

2. Correct SSN Not Located

  • Prepare a dictated letter unless other procedure specifies a form letter or stuffer may be used.

NOTE: Do not use the SSN anywhere in your reply, not even in the reference.

  • Tell the individual that the SSN he/she gave us is not assigned in his/her name or in the name for whom he/she is writing. Ask the individual to supply the necessary identifying information to enable us to check our records in an attempt to locate the correct SSN.
