RM 10211.375 How to Enter J1 and J2 Exchange Visitor Name and Date of Birth Data into the Enumeration System
A. Entering names
There are three documents issued to J1 or J2 non-immigrants that may have slightly different naming conventions based on the issuer of the document. For example:
I-94 (Arrival Departure Record),
DS-2019 (Certificate of Eligibility for Exchange Visitor (J-1) status), and
For exchange visitors without an Employment Authorization Document (EAD):
NOTE: Clearly different names between the required documents (e.g., I-94, DS-2019) are not acceptable, since the identity of the applicant cannot be confirmed.
For exchange visitors with an EAD:
If the exchange visitor legally changed his or her name in the U.S. after the DS-2019 or EAD was issued, you may assign the SSN in the new legal name.

B. Entering the date of birth
Enter the date of birth shown on the document having highest priority as evidence of age. For additional information on evidence of age, see RM 10210.250.
