A. Use of the Form SSA-5028

An applicant can use the SSA-5028 (Receipt for Application for a Social Security Number), when needed, to verify that he or she has applied for a Social Security Number (SSN). Form SSA-5028 is available in both English and Spanish.

The SSA-5028 is not a controlled form; it is simply a receipt. If the applicant does not receive his or her card within the period indicated on the SSA-5028, he or she must contact the field office and submit complete identifying information and supporting evidence. The field office employee needs this information to transmit a status query.

B. Preparation of the form

CAUTION: Before issuing Form SSA-5028, be certain that assignment of an SSN is proper. Verify all necessary evidence of age, identity, and U.S. citizenship or lawful alien status. In addition, do not issue the SSA-5028 to applicants whose SS-5's, (Application for a Social Security Card) have codes “N,” “X,” or “Y.” For these applicants issue Form SSA-5029, Application for a Non-Work Social Security Number.

    Enter the applicant's name and the date of the SS-5 and the above listed information and when received by the district office (DO) in the spaces provided.

    Enter the DO address and telephone number where indicated.
