GN 00308.220 Puerto Rico Vital Records

A. Process — Puerto Rico vital records

You can find information about requesting vital records from Puerto Rico (PR) at the National Center for Health Statistics or Departamento De Salud for information in Spanish.

Field Offices (FOs) in PR do not have direct access to the vital statistics database and are not able to verify or certify vital records events that occurred in their service areas.

Where applicable, include claimant's maternal and paternal surnames (e.g., Gonzalez Gomez) on all requests for vital records.

NOTE: For enumeration and claims purposes, SSA will only accept birth certificates from Puerto Rico with an issue date of July 1, 2010 or later. For acceptance of Puerto Rico birth certificates, see GN 00301.065.

B. Procedure — State sources — mail requests

Only the claimant may request their vital record. When necessary, SSA may assist the claimant with requesting information and relay a request on the claimant's behalf, see GN 00301.180 and GN 00301.185. The claimant should make a money order payable to Secretary of Treasury. Cash, personal checks, and bank checks are not accepted. NOTE: SSA may purchase copies of documentary evidence for any eligibility requirement of a Title II or Title XVIII program if the payment criteria in GN 00301.210A.3. are met. For payment criteria for documentary evidence for Title XVI, see GN 00301.210B.1.

Include a signed letter and a copy of a valid form of identification (driver's license, passport, or government-issued ID) from the claimant along with the appropriate information for the requested record. With the claimant's disclosure authorization, SSA offices may use the SSA-L706 to have a birth record returned directly to SSA, see GN 00302.070. To have requests for death, marriage, or divorce records returned directly to SSA, see the National Center for Health Statistics website.

The Central Office of the Demographic Registry has records pertaining to divorces registered as of 1941. Divorce certifications are NOT actual divorce decrees but an abstract of the information provided on the final court resolution, submitted to the Demographic Registry for official use only. Availability varies; not all divorce decrees are submitted to the Demographic Registry. In the case where a divorce decree does not appear registered in the Demographic Registry, a negative certification of divorce will be issued for the same cost.
