GN 01709.220 Additional Application Forms for Swiss Benefits
In addition to the Form SSA-2490-BK, an applicant for Swiss benefits must complete one of several special Swiss application forms. The forms to be completed will vary depending on the type of benefit claimed. For example, the Form CH/USA 2.3 (Application for Disability Insurance for Citizens of the United States of America Residing Outside Switzerland) is used to claim disability benefits from Switzerland. The method of filing will also vary depending on whether the applicant is a Swiss citizen.
The Swiss application forms are not taken in field offices (FOs). The method of filing and the type of form to be filed will vary depending on whether the applicant is a Swiss citizen and the type of benefits involved.
1. Applicant Is a Swiss Citizen
Forward the completed Form SSA-2490-BK along with any other forms and evidence to the Office of International Operations (OIO) at the address in GN 01702.310.
Advise the applicant that he/she must also file a separate application with one of the Swiss consulates or the Swiss Embassy.
When OIO receives the Form SSA-2490-BK, they will send a notice to the applicant explaining this requirement to file another application referring the applicant to the nearest Swiss consulate or the Swiss Embassy.
2. Applicant Is not a Swiss Citizen
Forward the completed Form SSA-2490-BK along with any other forms and evidence to OIO at the address in GN 01702.310.
Advise the applicant that OIO will mail another application form to him/her to complete and return to OIO.
If an applicant brings one of the Swiss application forms to an FO:
provide any necessary assistance in completing the form,
photocopy and certify any documents (evidence) to include with the Swiss claim, and
return the form (and certified photocopies) to OIO at the address shown in GN 01702.310.
