GN 01710.205 Preparing Claims Packages in DIO for Claims Filed Outside Switzerland

A. Introduction

Initial claims packages are prepared to request Swiss coverage records and to inform the Swiss agency about claims for Swiss benefits. (See GN 01709.205 - GN 01709.215 for the application and evidence required for the package.)

B. Procedure

1. Potential Entitlement to U.S. Totalization Benefits

If there is potential entitlement to U.S. totalization benefits, prepare an SSA-e2960-U3-SZ to request the Swiss coverage record.

2. Claim for Swiss Retirement or Survivor Benefits

If there is a claim for Swiss retirement or survivor benefits, prepare a claims package including:

  • Form SSA-e2960-U3-SZ;

  • evidence submitted for the Swiss claim; and

  • application Form CH/USA 2.1 or 2.2 (Application for an old age (or survivors) pension for citizens of the United States of America residing outside Switzerland) if required under the instructions in GN 01710.315B.

3. Claim for Swiss Disability Benefits

If there is a claim for Swiss disability benefits, prepare a claims package including:

  • Form SSA-e2960-U3-SZ;

  • evidence submitted for the Swiss claim;

  • medical evidence from DIO's files; and

  • application Form CH/USA-2.3 (Application for disability insurance for citizens of the United States of America residing outside Switzerland) if required under the instructions in GN 01710.315B.
