GN 01718.220 Completing the U.S.A./Sweden Transmittal/Request/Certification Form (SSA-2960-U3-SW)

A. Policy

Technicians in OIO and personnel in the American Embassy in Stockholm, Sweden communicate with the Swedish agencies via the electronic Form SSA-2960-U3-SW. The form is available via the Totalization Data Collection Program (TDCP).

B. When to use

The SSA-2960-U3-SW is used to:

  • transmit claims and related material to the Swedish agencies,

  • request information from the Swedish agencies, and

  • send information requested by the Swedish agencies.

C. Description of SSA-2960-U3-SW

The SSA-2960-U3-SW is an electronic form that is only available via the TDCP.

D. Procedure

1. General

To complete the electronic Form SSA-2960-U3-SW, access the electronic form via the TDCP and complete the following information.

2. Part I —Information About the Claim

  1. a. 

    ALWAYS complete A., B. and C.

    • If you don't know the SSN, enter “UNKNOWN” in B.,

    • If you don't know the PIN, enter “UNKNOWN” in C. and complete Part II A.1 and A.2.

  2. b. 

    Complete D., E. and F. to inform the SLAFU about a claim for Swedish benefits.

3. Part II —Certification of Data

  1. a. 

    Complete the appropriate section(s) to:

    • inform the SLAFU about claimants for Swedish benefits.

    • respond to requests for information from the SLAFU, and

    • inform the SLAFU about U.S. social security benefits sent to Swedish residents.

  2. b. 

    Mark “X” in the verified block(s) if the data has been used by SSA to adjudicate claims.

4. Part III —Transmittal of Information by U.S.

Mark “X” in the appropriate block(s).

5. Part IV —Request for Information from Sweden

Mark “X” in the appropriate block(s).

6. Part V —Remarks

Enter clarifying or additional information. Be concise and do not use jargon.
