GN 01719.220 Additional Applications for U.K. Benefits
In addition to the Form SSA-2490-BK, an applicant for U.K. benefits must complete one or more special U.K. application forms. The forms to be completed will vary depending on the type of benefit claimed.
The special application forms are not taken in the Field Offices (FO). After receiving a claim for U.K. benefits that was filed with SSA, the U.K. agency will mail the appropriate U.K. forms to the applicant. The U.K. agency will include instructions asking the claimant to complete the forms and mail them directly to the U.K. agency along with any additional evidence that may be needed.
If an applicant brings one of the U.K. application forms to the FO:
provide any necessary assistance in completing the form,
photocopy and certify any documents (evidence) the U.K. agency has requested the applicant to include with the U.K. claim, and
return the forms (and certified photocopies) to the applicant and advise him or her to mail them directly to the U.K. agency that sent them.
Reminder: Do not mail the U.K. application forms to OIO.
