In addition to the Form SSA-2490-BK, an applicant for French benefits must complete one or more special French application forms. The forms to be completed will vary depending on the type of benefit claimed.

The special application forms are not taken in the FO's. After receiving a claim for French benefits that was filed with SSA, the French agency will mail the appropriate French forms to the applicant. The French agency will include instructions asking the claimant to complete the forms and mail them directly to the French agency along with any additional evidence that may be needed.

If an applicant brings a French application form to the FO:

  provide any necessary assistance in completing the form(s),

• •

  photocopy and certify any documents (evidence) the French agency has requested the applicant to include with French claim, and

• •

  return the forms (and certified photocopies) to the applicant and advise him/her to mail them directly to the French agency that sent them.
