The Finnish agencies use their liaison form to

  transmit material to OIO or the RFSP

  send information to OIO or the RFSP, and

• •

  request information from OIO

  This form is printed on both sides of one page and is self-explanatory in most respects.

  There are no “verification blocks” on the form because all dates that are entered on the form have been verified by Finland.

  There is no blanket statement on the SF/USA2 which provides Privacy Act Authorization to release information to Finland. When requesting data/ information from SSA, Finland will always send along a separate signed authorization to disclose.
