A. When to use form KOR-USA 2

The NPC uses form KOR-USA 2 to:

  • transmit material to OIO or VARO

  • send information to OIO or VARO, and

  • request information from OIO

B. Exhibit- Form KOR-USA 2

 

C. Description of form KOR-USA 2

  • Part A contains identifying information about the worker and space to enter both the U.S. social security number and Korean national pension number. If the NPC is verifying the worker's date of birth, they will show that in this section.

  • Part B contains information about the applicant and will only be completed if the applicant is not the worker.

  • Part C contains information provided by Korea, including the protective filing date and the record of Korean coverage.

  • Part D contains a list of the types of information that the NPC might request from OIO along with the remark that the information is needed to process a claim under the agreement.

D. Procedure

In addition to the procedures for handling foreign agency liaison forms contained in GN 01704.400 - GN 01704.990, return to the NPC any request that does not indicate that it is needed to process a claim under the agreement unless

  • written consent to disclose is attached or

  • the file contains an SSA-2490 signed by the claimant.
