Prepare a SSA-2960 USA/PL 3 if a Polish certified coverage record is required and has not been requested.

B. Procedure – U.S. and Polish or Polish Claim Only

Prepare a SSA-2960 USA/PL3 to:

    Request the Polish certified coverage record if one is needed and has not been requested. Include a copy of the SSA-2960 USA/PL3.

    Send ZUS or KRUS the information or documentation they have requested.
