GN 01757.220 Additional Applications for Benefits under the Agreement with Brazil

A. Forms to complete for Brazilian Benefits

An applicant filing for Brazilian benefits must complete one or more Brazilian application forms, in addition to the SSA-2490-BK. The Brazilian form(s) will vary depending on the type of benefit.

B. Application process for Brazilian benefits

SSA FOs should not attempt to assist applicants in filling out Brazilian forms. However, SSA personnel may advise applicants that Brazilian application forms are available online at: http://www.previdencia.gov.br/servicos-ao-cidadao/todos-os-servicos/. After the applicant files a claim for Brazilian benefits with SSA, the Brazilian agency will mail any additional appropriate Brazilian forms to the applicant. The Brazilian agency will include instructions for the applicant to complete the forms and mail directly to the Brazilian agency along with any additional required evidence.

C. Procedure for filing an application for Brazilian benefits

If an applicant wishes to file for Brazilian benefits and brings the special forms for Brazilian benefits to the FO, take the following actions:

  • accept, photocopy, and certify the application and the special forms;

  • photocopy and certify any additional documents (evidence) the Brazilian agency requested that the applicant include with the Brazilian claim;

  • return the photocopied application and photocopied special forms to the applicant with any original evidence documents;

  • request an “Informational/Certified Earnings Record” (ICERS) as described in MS ICERS 001.004;

  • send the original Brazilian application and special forms, with any additional claims material such as certified photocopies of evidence documents to DIO at the following address:

  • Complete the MDW screen showing the date you sent the Brazilian claim to DIO. Show “Totalization” as the ISSUE.

Once DIO receives the claim, they send it to the Brazilian agency with the evidence and the worker’s U.S. coverage record. DIO annotates the MDW with the type of claim and date and sends the claim to Brazil.
