The Slovenian certified coverage record is the Form SI-USA 5 (Certificate of Insurance Periods). The SI-USA 5 is the only acceptable form for the certification of the Slovenian coverage record. Forward certified coverage records received in any other format to the Division of Training and Program Support (DTPS) for approval.
