GN 03905.055 Invalid Appointment Forms

A. When to use

Sometimes representatives submit notices of appointment (e.g. form SSA-1696 or "equivalent writing") before a claimant submits an application for benefits. When we receive these appointments, we will return them unprocessed together with the following language available in the Document Processing System (DPS). Technician can find the full notices by title on DPS.

B. Exhibits

1. Appointment Documents Returned - No Pending Claim

We are returning the appointed representative forms you submitted for [INSERT: Claimant’s Name]. We could not process the forms because [INSERT: Claimant’s Name] does not have a matter pending before us, such as a post-entitlement issue, or a pending claim on file or appeal.

If your client intends to file a claim for benefits, an appeal or other matter, or you submitted an iClaim on their behalf, please re-submit your documents once this matter is filed.

REP040 - optional: The representative’s identifying information (e.g., name, RepID)
