GN 04440.241 Correcting Unsigned Consultative Examination (CE) Reports

A. Policy for an unsigned CE report technical corrective action (TCA)

An unsigned CE report TCA exists when the determination is unfavorable or less than fully favorable, and a CE report used in the determination is either:

  • unsigned; or

  • signed by someone other than the physician who actually performed the CE.

In these situations, do not initiate any corrective action; rather, assume that the adjudicating component is obtaining the appropriate physician’s signature post-adjudication per DI 22510.020C.2.

B. Procedure for correcting unsigned CE reports

When the determination is unfavorable or less than fully favorable, and an unsigned CE report deficiency exists, prepare an informational SSA-847-U3 (SSA Request for Case Action) and return the case to the adjudicating component:

  • annotate the return to indicate that no deficiency is being charged;

  • advise the adjudicating component of the TCA and ask them to obtain the appropriate physician's signature;

  • request that the adjudicating component add the signed CE in the folder;

  • instruct the adjudicating component not to return the case to the review component, since follow-up review is not necessary; and

  • advise the adjudicating component to release the case to the appropriate processing component to effectuate the determination and to release the notice.
