RS 01404.215 Request for Wage Information - Members of Army and Air National Guard

A. Procedure — servicing FO

1. General

Request wage evidence from the State Adjutant General for National Guard service in all cases, including those where the alleged service was more than 5 years prior to the year of filing.

2. State adjutant general

Forward request:

  1. a. 

    through PSSO in RS 01404.315;

  2. b. 

    via Form SSA-562-U3;

  3. c. 

    using sample letter in RS 01404.320.

3. Specific wage amounts unavailable

If response from State Adjutant General does not provide specific wage amount information, establish wages according to instructions in RS 01404.225.

B. Procedure — PSS0

1. General

Obtain wage evidence and return it to servicing FO as promptly as possible.

2. Changes of address

Advise the OISP/OEEMP, 6401 Security Blvd., 2514 RMB Bldg., Baltimore, MD 21235 of change of address.
