The name “PHS” may not always appear in the employer name. PHS employers without PHS in the name include:

  Centers for Disease Control, Atlanta, Georgia;

  Robert A. Taft Sanitary Engineering Center, Cincinnati, Ohio;

• •

  National Institutes of Health, Bethesda, Maryland;

• •

  Rocky Mountain Laboratory, Hamilton, Montana;

• •

  U.S. Quarantine Station, Staten Island, New York.

The Department of Health and Human Services and the Coast Guard may also report service by a PHS officer. Assume that any employment shown is as a PHS officer.
