When it is necessary to request medical information from SSA disability files for
VR purposes, the following procedures should be followed:

Query to determine the location of the folder.

2.
Prepare a letter of request (in duplicate) to the appropriate office and identify
the case by social security number, disabled person's name and date of birth.

3.
Where medical information from more than one case is requested in the same letter,
list the cases (double spaced) in social security number order.
