DI 30001.410 Technical Review of Form SSA-831-U3 (Disability Determination and Transmittal)

A. Field Office (FO) and Disability Determination Services (DDS) Responsibilities

The FO and DDS are responsible for accurately completing the appropriate blocks on the SSA-831.

1. Items 1 - 14

The FO is responsible for the completion of items 1-14 of the SSA-831-U3.

2. Items 15 - 34 and item 36

The DDS completes items 15-34 and item 36 of the SSA-831-U3 to record the disability determination.

B. Procedure

1. Items 1 - 14

  1. a. 

    DDSs should verify the information in items 1-14 for accuracy as specified in DI 26510.001- DI 26510.005.

  2. b. 

    If there is a significant error in items1-14, notify the FO of the error.

  3. c. 

    Resolve discrepancies involving SSN, name, and type of claim before DDS systems input is made.

2. Items 15 - 34 and item 36

  1. a. 

    When reviewing items 15-34 and item 36, follow the guidelines in DI 26510.000 and DI 27015.000 for completing the SSA-831-U3.

  2. b. 

    In item 34 (Remarks): For cases that are Certified Electronic Files (CEF), DDS QA updates the DDS worksheet with an entry indicating that a claim was reviewed by the QA unit. For non-electronic cases, verify whether “DDS QA” has been entered whenever DDS QA has reviewed the case.
