DI 30005.240 Correcting Unsigned Consultative Examination Reports

A. Definition of an unsigned consultative examination report technical corrective action

An unsigned consultative examination (CE) report technical corrective action (TCA) exists when the determination is unfavorable or less than fully favorable, and a CE report used in the determination is either:

  • Unsigned; or

  • Signed by someone other than the physician who performed the CE.

B. Policy for correcting unsigned CE reports

When the determination is unfavorable or less than fully favorable, the review component returns the case to the adjudicating component and request correction of the TCA pre-effectuation.

NOTE: When the determination is fully favorable, and the case is otherwise correct, the review component assumes that the adjudicating component will obtain the missing signature on the CE report per DI 22510.020C.1. and forward it as trailer mail.

C. Adjudicating component procedure on unfavorable or less than fully favorable determination

When the review component returns a determination to obtain the missing examining physician signature on the CE report:

  • Obtain the signature of the physician conducting the CE;

  • Place the properly signed CE report in file;

  • Clear the case to the appropriate component; and

  • Do not return the case to the review component unless they instruct you to do so.
