DI 40510.180 Processing Appeal Requests - Premium Medicare Cases

A. Policy

If an individual is denied Medicare coverage and subsequently files a request for reconsideration or hearing, the FO obtains an SSA-561-U2 or HA-501-U5 and any additional evidence/material the claimant offers and sends it to ODIO/ PSC for processing.

B. Procedure - reconsideration

1. Non-medical issue

For reconsideration of a non-medical issue:

  1. a. 

    Associate the SSA-561-U2 and any related material with the folder.

  2. b. 

    Forward to a reconsideration DE for processing.

2. Medical issue

For reconsideration of a medical issue:

  1. a. 

    Associate the SSA-561-U2 and any related material with the folder.

  2. b. 

    Forward to the appropriate DDS for processing.

C. Procedure - hearing

If a hearing is held, associate the HA-501-U5 and any related material with the folder and send to the appropriate hearing office.
