DI 42548.006 Inquiries

A. Policy

Responders (“walk-ins”) may inquire about their Hyatt claims by contacting the local FO or OHA hearing office in person, in writing or by telephone. In addition, individuals may inquire by contacting any SSA office by telephone. Individuals who lost the reply form, or never received one may complete a Hyatt “Application for Class Membership” (DI 42548.095, Exhibit 3) requesting review.

B. Process

Applications obtained by the FO or OHA are forwarded to:

The data from the applications is entered into CATS. Because OHA hearing offices do not have the necessary systems capability OCO should verify the claimant's SSN, claim number, and BIC/MFT via numident, DDBQ, MBR, SSR, etc., and annotate any current entitlement on all applications forwarded by OHA. OCO associates the applications with the systems generated CATS alert-query package and routes the claim per DI 42548.008.

If an individual contacts the PC or OCO instead of the FO about their Hyatt claim, the PC or OCO will follow DI 42548.006C.

C. Procedure

1. CATS

Query CATS to determine if the individual has been identified as a Hyatt class member and whether the response form has been received and an alert has been generated.

NOTE: If the PC cannot query CATS directly, they should contact the Regional Office or OCO when it is necessary to access CATS to secure information relating to claimant's response.

2. Identified as a class member

Take the following action when an inquiry is made, and the individual has been identified as a class member.

  1. If a response for the individual is shown on CATS, send the inquiry to OCO at the address shown in DI 42548.006B., under cover of a transmittal annotated “Hyatt Response shown on CATS.”

  2. If the individual has been identified, but no response has been recorded on CATS, update CATS. (See MSOM CATS 010.001.) Send the inquiry to OCO at the address shown in DI 42548.006B., under cover of a transmittal annotated “Process Inquiry as Hyatt Responder, No Response on CATS.”

3. Not identified as a class member

If the individual has not been identified as a Hyatt class member, consider the inquiry as a request for review under Hyatt.

  1. If an individual calls the PC or, OCO prepare an SSA-5002 (Report of Contact) containing the individual's name, SSN, address, and telephone number, and annotate the SSA-5002 “Application for Hyatt Review.” (If the individual is unclear about whether they wish to apply for class review, treat the inquiry as a request for review. Do not try to dissuade anyone from requesting review.)

    • Update CATS (see MSOM CATS 010.001).

    • Send the SSA-5002 to OCO at the address shown in DI 42548.006B., under cover of a transmittal annotated “Process Inquiry Hyatt Responder.”

  2. If a written inquiry is received in OCO:

    • Verify SSN;

    • Enter data into CATS.

  3. If a written inquiry is received in the PC send the inquiry to OCO at the address shown in DI 42548.006B. under cover of a transmittal annotated “Process inquiry as Hyatt class application, Claimant Not on CATS.”
