DI 45001.040 Multiple ESRD Entitlement Periods

A situation may arise in which it will be necessary to consider two or more ESRD entitlement periods at the time of initial adjudication. For instance, an individual may cease a course of dialysis, remain without dialysis or transplant for one year, then begin a second course of dialysis. In these situations, prepare a separate SSA-892-U3 for each entitlement period while keeping in mind the 12 months retroactivity limitation.

It should be noted that even when the individual is a disability beneficiary, an SSA-892-U3 for ESRD Medicare entitlement is still needed.
