Language shown in the exhibit notices (DI 45001.076-DI 45001.104) is provided for typical ESRD situations requiring notification. However, the language
provided should be modified as needed to accommodate the less common situations. In
addition, when the claimant or his/her representative has a foreign mailing address,
the case will be processed as usual except for notification. Prior to release of the
notice, modify the appeals and referral language to assure that the language is appropriate
for a foreign reader.
