DI 45010.005 Termination Events for Medicare Coverage (R-HI, R-SMI) Based on ESRD

See HI 00820.030.

Medicare based on ESRD ends with the earliest of the following events:

1. A. 

   The date of death of the individual, or

2. B. 

   The last day of the 36th month after the month in which the individual receives a kidney transplant, unless on or before such last day he/she begins a course of dialysis or receives another transplant; or

3. C. 

   The last day of the 12th month after the month in which the individual terminates dialysis, unless on or before such last day, he/she receives a transplant or begins a new course of dialysis.
