HI 00630.080 Procedure for the Issuance of an HI or SMI Card

To have an HI or SMI card issued, send a copy of the claimant's request under cover of an OF-41 to OCRO, HI Reconciliation Branch, 2-N-6 Annex, asking that an HI/SMI card be issued. Make sure that the claimant's full name, address, and claim number appears on the correspondence. Where the claimant has not specifically requested a card but it appears the claimant may need one, send an informal memorandum to OCRO requesting the issuance of the appropriate card.
