Whenever an equitable relief condition is present on an uninsured claim with premium-HI
involvement, relief must be considered and may be granted whenever the retroactive
period in question (whether an enrollment, termination, or nonbilling situation) exceeds
2 months.
Inform the claimant of his right to request relief. Also, diary the case 45 days to
prepare a manual bill if there is no response from the claimant.
