HI 00901.165 Health Insurance Card Inquiries - RRB Involvement and FO Action
This table describes the action necessary for FO development of beneficiary inquiries concerning the nonreceipt, replacement, or correction of cards issued by RRB.
|1 ||Telephone the local RRB office so that an RRB card can be issued. |
Provide RRB with all pertinent data as follows:
If the inquiry involves loss or nonreceipt of card, provide beneficiary name, RRB claim number, and address (indicate if this is a recent change).
If the inquiry involves correction of information on card, provide beneficiary name, RRB claim number, and information to be corrected.
|3 ||Inform the beneficiary that the new card will be mailed by RRB. |
