The SSA-L823 is used in the DDS's, ODO and the DO's to deny auxiliary claims when number holder is found not disabled. The PSC's can use the SSA-L823. Examiner will designate “823” in block 29 of the SSA-831. Name and address must be taken from application form in file. Prepare two form notices - one for the claimant and one for the claimant's file.

  For other than child's benefits, type a period after the word “benefits.” If a representative payee is involved, add the word “for” and the name of the claimant. If more than one child claimant is involved, type an individual notice for each child. If it is necessary to send the payee and children Form SSA-L823, prepare a separate notice for the payee and a separate notice for each of the children.

NOTE: If no other type of benefit is pending use paragraph RG07.
