We are writing to let you know that we are stopping your Social Security benefits in

How We Can Pay You
We can only continue your benefits if you have a child in your care, under age 16 or disabled, who is entitled to Social Security benefits.

Other entitled beneficiaries will continue to receive the same benefit amount. There are six fill-ins. If the foreign referral paragraph is needed, indicate 3901D on the SSA-573 along with the appropriate fill-in.
