We are writing to let you know that we will stop your Security benefits in 15 days after you receive this letter if we do not hear from you. We have not been able to get in touch with you to see if you should still receive benefits. If we do not hear from you in the next 15 days, we will assume you did not receive this letter either.

 

This is The Last Time We Will Try to Contact You

We cannot pay you benefits because the Form SSA-1588 “Beneficiary Recontact Report” we sent you was returned to us by the Post Office. We need the information requested on this form to decide if you can receive benefits again. This is the last time we will try to contact you before we stop your benefits.
