12000 INTRODUCTORY PARAGRAPH

This is in response to your recent inquiry about payments to you under the Supplemental Security Income program.

12001 FOR USE FOR INDIVIDUAL WHO IS NOT A U.S. CITIZEN OR HAS NOT BEEN ADMITTED FOR PERMANENT RESIDENCE

To be eligible for these payments, an individual must be a citizen of the United States or have been lawfully admitted for permanent residence in this country. Our records indicate you do not meet this requirement.

12002 FOR USE FOR INDIVIDUAL WHO IS NOT A RESIDENT OF THE U.S.

To be eligible for these payments, an individual must live in one of the 50 States or in Washington, D.C. Our records indicate you do not meet this requirement.

12003 FOR USE FOR AN INDIVIDUAL WHOSE PAYMENTS ARE STOPPED BECAUSE OF DEPARTURE FROM THE U.S.

To continue to receive these payments, an individual must continue to reside in one of the 50 States or in Washington, D.C. Our records show you  (have been)  ,   (will be)  outside the United States for a full calendar month and, therefore, not eligible for benefits beginning   (first full calendar month of absence)  .

12004 FOR USE FOR INDIVIDUAL WHO IS NOT AT LEAST AGE 65 AND DOES NOT INDICATE HE IS BLIND OR VISUALLY IMPAIRED OR DISABLED

To be eligible for these payments, an individual must be at least age 65, disabled, or blind. Our records show you were born  (date of birth)  . Therefore, you are not age 65, and there is no indication that you are disabled or blind.
