    A. Purpose

      This Emergency Message (EM) provides Field Office (FO) reminders on processing Disability Determination Services (DDS) and Hearings level specific documents in WorkTrack.


    B. Background

      FOs receive certain forms and evidence requested by the DDS or a Hearing Office in WorkTrack by various submission methods (e.g., Upload Documents, Mail Centralization and Digitization (MCD), Office Mail, and Online Fax).

      Technicians and managers review these documents and manually transfer them to eView. This EM sets forth reminders to help ensure that FO technicians route forms and evidence to the DDS and Hearings Offices expeditiously.


    C. Instructions

      FO technicians should expedite processing of forms and evidence intended for the DDS or Hearings Offices via the electronic folder (i.e., eView). These forms include:
        · SSA-3368 - Disability Report Adult

        · SSA-3369 - Work History Report

        · SSA-3373 - Function Report - Adult

        · SSA-3441 - Disability Report Appeal

        · SSA-454 - Continuing Disability Review Report

        · SSA-765 - Response to Notice of Revised Determination

        · SSA-769 - Request for Change in Time/Place of Disability Hearing

        · SSA-773 - Waiver of Right to Appear - Disability Hearing

        · SSA-789 - Request for Reconsideration - Disability Cessation Right to Appear

        · SSA-827 - Authorization to Disclose Information to the Social Security Administration


      If no field action is required, process the forms and evidence to completion and send them to the DDS/Hearings Offices without WorkTrack assignment. FO management or the technician responsible for profiling WorkTrack should:
        · Verify the case is in either the DDS or the Hearings Office, then transfer the form or evidence from the list above to eView following the instructions below. Do not assign these items to an FO technician.

        · Update the form or evidence type as necessary if the document submitted should not be routed to eView, and assign the item in WorkTrack following normal procedures.


      NOTE: “Process a document” in WorkTrack, refers to the action of updating the status of a document from pending to complete. Update the status either by annotating a final disposition to the document or by uploading the document to the Non-Disability Repository for Evidentiary Documents (NDRED), eView, or Paperless from within WorkTrack.
        1. Steps to process a document
            a. Review the image quality. If any changes to the page(s) are required, such as page rearrangement or page deletion or rotation, use the EDIT PROFILE button. Once you make corrections to the page(s), use the “Save & Process” button on the “Profile” screen to navigate back to the “Process" screen.

              NOTE: Do not select “other” as a form type, unless there are no applicable options.


            b. Select an action on the process screen
                i. From the action field drop down list choose the “Select eView” option.

                ii. You may receive a “Repository Form Selection” box if the Form Type does not match a form type for eView. Review the form types offered in the drop-down menu and select the appropriate one.


                  NOTE: Do not select “other” as a form type, unless there are no applicable options.
            c. Annotate any remarks as appropriate.

            d. Select the “Submit” button.

      NOTE: If a Field Office receives evidence on an existing paper folder case they should review the following policy references for handling: DI 81010.125, DI 81010.150, and GN 00301.286.


    D. Misrouted Documents


      The DDS and Hearings Offices will notify the FO of misrouted forms following existing policy and procedures (i.e., assistance requests) for any necessary FO action.
