Background
This transmittal updates our instructions to include a link to the Oklahoma State Department of Health website that provides the current address, cost and type of payment for obtaining copies of vital records. These revisions do not change or introduce new policy or procedure.

Subsection B.
We removed the information in B.1-B.3 and replaced with a link to the Oklahoma State website for technicians to access the current address, cost and type of payment along with the most up-to-date information from Vital Records.
We expanded the word FO to Field Office for the first use of the abbreviation.
We removed the B.4 Remarks section and placed this information under Section B- Procedure. We added a link to the Oklahoma State Department of Health website that includes the current mailing address to send a request to verify the validity of a birth record.
We updated the last sentence in the 4th paragraph "Send only requests for verification to the following address" to "Only requests for verification should be sent to the following address".
Marriage and divorce records are available in the county where the marriage license was issued or the divorce was granted.
To obtain a vital records with certified birth /death records, technicians must write directly to the State at the address provided on the Oklahoma State Department of Health website.
The address, cost, and type of payment along with the most up-to-date information from Vital Records can be found on the Oklahoma State website . In addition, you can find EIN information here . The parallel Field Office (FO) cannot follow up on any requests.
If SSA is paying for the record (i.e., requirements for SSA purchase in GN 00301.210 are met), include a third party draft payable to “Oklahoma State Department of Health.” A valid, legal photo I.D. is required from the applicant or the individual representing the applicant for the issuance of a certified birth or death certificate. SSA FOs should follow procedures for identification requirements in GN 00308.001B.3.
Special procedures have been established for SSA offices to verify the validity of a birth record submitted by the applicant at a reduced search fee of $5. FOs must complete items 1, 3, and 4 of form SSA-L706-F3 (05/2005), including the applicant’s signature in the release section of item 1 in lieu of ID of applicant. Attach a legible copy of the birth certificate to be verified. Include a stamped, self-addressed return envelope and a third party draft of $5 payable to “Oklahoma State Department of Health.” Only requests for verification should be sent to the following address.
