A. Purpose
The purpose of this Emergency Message (EM) is to inform Social Security Field Office (FO) technicians about new procedures for obtaining the necessary forms when completing a Title II request for EXR.

B. Background
The prior procedure for processing a Title II request for EXR was to obtain the claimant’s acknowledgement of Medicare enrollment (if not currently receiving extended Medicare coverage) on form SSA-795.

C. New Procedure
Effective immediately, technicians will no longer obtain a separate form SSA-795. Form SSA-371 (Request for Reinstatement – Title II) now includes a Medicare enrollment acknowledgement statement. Technicians should continue to obtain all other forms listed in
DI 13050.045.

Direct all program-related and technical questions to your Regional Office (RO) support staff using vHelp or Program Service Center (PSC) Operations Analysis (OA) staff. RO support staff or PSC OA staff may refer questions, concerns, or problems to their Central Office contacts.
