Background

OMP plans to migrate the impairment code POMS, which is an OMP policy area, from DI 26510.015 (an OVEPP-controlled POMS section) to new POMS sections controlled by OMP, DI 33526.001 through DI 33526.100.

In addition to moving the impairment codes, OMP would be making changes to some of the current impairment codes. Also, changes to some of the current impairment descriptors would also be made for purposes of plain language. Ultimately, this move would allow OMP to readily update the impairment codes as needed. There are also minor changes being made to the digestive and skin impairment codes to align with the new listings for those body systems effective 10/6/2023.

Summary of Changes

DI 33526.001 Background on Impairment Code Lists

This new section provides the impairment codes in numeric sequence to make it easier to identify the descriptor for a specific code.

DI 33526.001 Background on Impairment Code Lists

A. SSA's use of impairment codes on disability determination forms

SSA uses a standardized set of four-digit impairment codes on the SSA-831 (Disability Determination and Transmittal), SSA-832 (Cessation Or Continuance of Disability or Blindness Determination and Transmittal - Title XVI), and SSA-833 (Cessation or Continuance of Disability or Blindness Determination and Transmittal - Title II). These codes are loosely based on ICD-9 coding. Generally, the highest level code that is appropriate is used. When that code is only three digits, a zero is added as the fourth digit.

Each code identifies the underlying cause of the person's disability, and the disabling manifestations. The codes most relevant to the determination must be used.

B. Using the tables of impairment codes

For the complete list of impairment codes in numerical order, including the body system code, see DI 33526.100. For each of the 15 body systems in the Listings of Impairments, we provide:

  • A list of codes to be used when a specific Part A (adult) listing is met or equaled;

  • A list of codes to be used when a specific Part B (child) listing is met or medically equaled;

  • A general list of codes to be used when the impairment does not meet or medically equal a Part A or Part B listing;

  • Reminders for selecting codes within the body system.

For the immune body system, we also provide a list of codes to be used to indicate the secondary impairment when the primary impairment is human immunodeficiency virus (HIV) infection.

We also include a section for impairments or situations that do not align with a specific body system as well as a section with all codes in numerical sequence.

  • Use impairment code 3350 only if the established diagnosis is amyotrophic lateral sclerosis (ALS). See DI 24580.020D3.

  • Use impairment code 3358 if the established diagnosis is a motor neuron disorder other than ALS, for example, progressive bulbar palsy, pseudobulbar palsy, spinal muscular atrophy (SMA), or primary lateral sclerosis (PLS).

  • Do not use impairment code 3480 for a medically induced coma. Use the impairment code for the underlying disorder causing the need for a medically induced coma.

D. Reminders for choosing an impairment code for mental disorders

  • Do not use impairment code 3150 if the child is over age three at the time of adjudication.

  • Do not use impairment code 3195 or 3152 if the allowance is based on meeting or equaling 12.05 or 112.05. If the allowance is based on meeting or equaling an intellectual disorder, use code 3180.

  • Impairment codes 3030 and 3040 should only be the primary impairment code when the case is a denial because drug addiction or alcoholism (DAA) is material . When the case is an allowance because DAA is not material, enter the mental or physical impairment found to be disabling, absent the effects of DAA, as the primary diagnosis, and enter impairment code 3030 or 3040, as appropriate, as the secondary diagnosis.

  • If the cancer meets or equals 113.03, use the impairment code for cancer of the affected body part. For example, if the cancer is in the skeletal system (e.g., bone), use 1780.

  • If the cancer meets listing 13.05, 113.05, or 13.28 based on bone marrow or stem cell transplant, use 0420 as the primary impairment code. Use the code for the specific cancer type as the secondary impairment code.

NOTE: For additional information on how cancer is evaluated under the listings, see DI 34005.113.

  • Do not use 0440 as the primary impairment code for a favorable determination. Because the primary impairment code is the basis for the allowance, this would mean the determination was based on HIV infection which has no symptoms, signs, or manifestations.

  • Use impairment code 1390 for any opportunistic infection that is associated with symptomatic HIV infection. This includes any bacterial, viral, parasitic, or fungal disease. Examples of diseases under this code include: toxoplasmosis, candidiasis, salmonella bacteremia, herpes simplex, cytomegalovirus, and syphilis.

  • If DI 33526.070D does not include an impairment code for the HIV manifestation, and impairment code 1390 would not be appropriate, use a code from the list of impairment codes for the appropriate body system.

DI 33526.075 Impairment Codes for Low Birth Weight and Failure to Thrive

Use the impairment codes in this section to identify Low Birth Weight (LBW) and Failure to Thrive (FTT). See the reminders in DI 33526.075C for codes with an asterisk (*).

NOTE: Low Birth Weight and Failure to Thrive does not have any listings in Part A.

A. When LBW or FTT meets or medically equals a Part B listing

Use the appropriate impairment code from the following table if LBW or FTT meets or medically equals a Part B listing:

LBW and FTT Impairment Codes for Part B Listings

Part B Listing

100.04B

*7649

100.05

*7830

B. When LBW or FTT does not meet or equal a listing

Use the appropriate impairment code from the following table if LBW or FTT does not meet or medically equal a listing:

General Impairment Codes for LBW and FTT

C. Reminders for choosing an impairment code for LBW or FTT

  • Do not use 7649 or 7650 for a continuing disability review (CDR) continuance. [LBW is no longer considered a medically determinable impairment (MDI) after the infant attains age one.]

  • Do not use 7830 if the child has attained age three before the date of the determination. Use an impairment code in the body system that is primarily affected.

DI 33526.080 Impairment Codes for Special or Other Situations

Use the impairment codes in this section to identify special or other situations. If a code in the table has an asterisk (*), see the reminders in DI 33526.080B.

B. Reminders for choosing an impairment code in special or other situations

  • Use 20 to identify the Body System in Item 16A of the SSA-831 or Item 21 of the SSA-832 or SSA-833.

  • Do not use 2480 as the primary diagnosis in Item 16A of the SSA-831 or Item 21 of the SSA-832 or SSA-833 if the decision is an allowance or a continuance.

  • Do not use 0000 as the primary diagnosis for continuing disability reviews (CDRs) in Item 21 of the SSA-832 or SSA-833.
