Background

The Department of State (DoS) updated their process for verifying certain DoS documents. Policy worked with (DOS) and with our Operations component to update SSA’s business process and POMS as well as the Form SSA-377 that is used for these requests.

Summary of Changes

RM 10213.025 Exhibit of SSA Verification Request for U.S. Passport Records (SSA-377)

1.Title: Modified titled "Exhibit of Verification Request for Department of State (DOS) Records (Form SSA-377)" .

1st paragraph added language "Verification Request for Department of State (DOS) Records” is used to request the DOS verify U.S. passports, passport cards, form FS-240 or form DS-1350. For detailed instructions on completing the SSA-377, refer to RM 10213.010.

2nd sentence removed the reference to "Forms Inventory Control System" (FICS) because it no longer exists.
