Delaware requires a copy of the requester's official photo identification (driver's license, State ID, or work ID) for vital record requests. A parent's identification is needed for a child. If an FO employee is the requester, see GN 00308.001B.3. for acceptable forms of identification.
Can FO verify, certify or obtain free copy of birth/death record? Yes, but attempt to verify through the Electronic Verification of Vital Events (EVVE) before contacting the FO.
FO will verify FAX request accompanied with a photocopy of the birth certificate. Requests are returned by mail. The FAX number is 302-736-3039. Verifications are made every Friday.
