The minimum data required for effective processing of an SSN application are:

  Full legal name to be shown on card;

  Name at birth and name shown on most recent SSN card, if different;

• •

  Citizenship or alien status;

• •

  Date of birth;

• •

  Place of birth;

• •

  Sex code (M or F);

• •

  Full name of one parent, preferably the mother's maiden name, if known (see 10205.145); and

• •

  Complete, valid mailing address.

Occasionally, an individual cannot provide the minimum data needed to complete an application for a Social Security card or cannot provide the evidence required to assign an SSN or to issue a replacement card, e.g., the applicant is homeless, has amnesia, or is mentally impaired.
