GN 01070.805 OCO Signature Requirements

A. Overview of signature requirements

    Certain actions involving the correction of SSA’s record of a client’s earnings require that statements or forms be signed by the client.

  • •

    Personal identifiable information from the record of an individual may be disclosed with the individual’s written content (GN 03305.001).

B. Signature required

The client’s request to SSA to correct their record must be in writing and signed by the client. (See GN 03305.001B. for related policy.)
