GN 01728.210 Preparing Claims Packages in OIO for Claims Filed in Portugal

A. Introduction

Initial claims packages are prepared to request Portuguese coverage certifications and to send information to the Portuguese agency.

B. Procedure

Follow these steps to prepare a claims package.

1. U.S. Claim

  • •

    Prepare a claims package if a Portuguese coverage certification is required and has not been requested by the FSP in Lisbon.

  • •

    Include Form SSA-2960-U3-PE in the package.

2. U.S. and Portuguese or Portuguese Claim Only

Prepare a claims package if:

  • •

    the Portuguese coverage certification is required and has not been requested by the FSP; or

  • •

    the Portuguese agency has requested information or documentation.

Include the following in the claims package:

  • •

    Form SSA-2960-U3-PE; and

  • •

    information or documentation requested by the Portuguese agency.
