A. Procedure – U.S. claim

Prepare a claims package if a Chilean coverage record is required and has not been requested.

Include Form SSA-2960-CHI in the package.

B. Procedure – U.S. and CHILEAN or Chilean claim only

Prepare a claims package if:

  • •

    A Chilean coverage record is required and has not been requested, or

  • •

    The Chilean agency has requested information or documentation.

    Include Form SSA-2960-CHI and the information or documentation requested by the Chilean agency.
