Statements signed by the custodian of the employer's records based on those records are acceptable as primary evidence if they are on a Form SSA-7011-F4, SSA-1002-F3, SSA-1003-F3 or have the same essential information as appears on any one of these forms.

The statements need not be addressed to SSA but should give complete identifying information relating to the custodian.

Obtain a signed statement from the custodian of the employer's records when such records are in the possession of the courts, bankruptcy judges, receivers, attorneys, accountants, succeeding employers, or others, and such statement is readily available.
