RS 02803.400 Processing Center Notification of Misuse
To ensure that critical case procedures are being used properly, processing centers
are charged with providing feedback to the Regional Office (RO) noting all deficiencies.
Notify the appropriate RO whenever CPS is used improperly by the FO, or the FO erroneously
telephones or e-mails the PC for assistance. Refer to DOORS for RO's address.
Misuse of critical case procedures include the following:

Improper critical case referral (i.e., case is excluded from the critical case definition).

CPS was used when a CPS exclusion exists.

3.
Incorrect field input on CPS action.

4.
Insufficient information furnished in a CPS input.
Obtain a copy of the erroneous document (CPS output, telephone summary or e-mail).
For each error, prepare a Form SSA-2711, Notice of Misuse of Critical Case Procedures.
Explain the deficiency on the SSA-2711 (cite a procedure reference, if applicable).
Attach the SSA-2711 to the copy of the erroneous document and forward them to OSA
for referral to the appropriate RO (i.e., the RO in which the FO that committed the
error is located). (Refer to the DOORS for Regional Commissioners' addresses.) Send
Forms SSA-2711 to the Operations Analysis Staff (in the PCs) or the Operations Support
Staff (in ODO); the Operations Specialists will accumulate the Forms SSA-2711 and
send them to the ROs on a monthly basis no later than the 15th of each month.
In addition to using a SSA-2711, if the PC believes it is essential for the FO to
be advised quickly of the error (e.g., to prevent an overpayment), notify the FO by
telephone or e-mail. If the name and telephone number of the FO contact is not available
(e.g., on a CPS error), telephone or e-mail the Management Support Specialist in the
FO.
